Submit a vehicle identity check (VIC) application form (VIC1)
Form VIC1 must be submitted to the Vehicle and Operator Services Agency (VOSA) before a VIC can be carried out.
How to use this service
To apply, use the link below to download the application form VIC1.
To access the form you will need the latest version of Adobe Reader. Information on how to download this for free is available on the right pane of this page.
You can submit your completed form online provided you intend to pay by either credit or debit card or have a VOSA Pre-funded Account. VOSA cannot currently collect your card details online and will contact you for this information once your VIC1 application form has been received.
You will be notified of an appointment once VOSA has processed your application. Full instructions and the ‘Terms and conditions’ are provided on page three.
